On your first visit, we will begin a permanent record. We will ask you
to complete some brief forms. Please help us stay on time by arriving a
few minutes early to complete the forms.
We want you to feel comfortable about calling us with any questions or concerns. We are here to serve you – from reception through physician. Please feel free to talk with your nurse and doctor.
